Preparing for Your Visit
A little preparation before your appointment can help make check-in easier and give our team the information needed for your care.
When arriving at our Allen, Texas office for your first consultation, please bring the appropriate identification, insurance information, medical information, and any relevant records that may pertain to your visit.
If your appointment involves insurance coverage, co-payments are due at the time of service.

A current driver's license or other valid photo ID.
Bring your current insurance information when applicable.
Include prescription medications, over-the-counter medications, vitamins, and supplements.
Bring any referral documentation required for your visit.
Pathology reports, diagnostic reports, or other records related to your consultation.

Insurance Plans
North Texas Plastic and Reconstructive Surgery works with many major area insurance plans. Coverage varies by plan, procedure, and individual benefits, so we recommend confirming your current coverage before treatment.
